Facelift (Rhytidectomy)

A Facelift (rhytidectomy) is a surgical procedure designed to remove loose skin from your face and neck and reduce the signs of aging in the facial area, giving you a more youthful appearance. The operation involves reducing the creases from the nose and mouth, the slacking jaw line, sagging jowls and fat deposits around the neck by removing excess skin and fat, tightening the underlying muscles and then redraping the skin on the neck and facial areas.

Good candidates for facelift surgery are those who:

• Have face and neck skin that has begun to sag, but still possess good skin elasticity
• Have strong and well-defined bone structure
• Experience the loss of a well-defined jaw line from jowls
• Have deep wrinkles in the cheeks
• Have loose skin, wrinkles or excess fatty tissue in the neck

A facelift is performed by making an incision around the natural contour of the ear, around the earlobe and inside the hairline. This helps conceal scar visibility. An incision can easily be masked by makeup or hair. A small incision may be made under your chin to deflate and tighten the structures in your neck. Dr. Valauri also offers the Short-scar facelift which can achieve improvement with less visible scarring than with the conventional facelift incisions. After the surgery you may have to have a draining tube to allow excess fluid to empty from the incision. Your bandage will stay in place for 1-2 days. You may experience some increased swelling in the first 24 to 48 hours after surgery. Most bruising will subside in about 2 weeks. Some numbness may occur initially, but resolves with time. The stitches will remain until your one week follow-up visit.

Since each individual has his or her own unique facial features, physical reactions and healing abilities, the type of facelift performed and final results will vary with each patient. Facelift surgery can be combined with eyelid surgery and or a forehead and brow lift to provide a total facial rejuvenation. Additionally, various implants for the cheek or chin may be suggested to enhance your overall aesthetic balance.

Lip Enhancement

Lip enhancement can be accomplished with injectable fillers, implants and surgery. Down turned corners of the mouth, thin lips or overly thick lips can be improved by a variety of minor surgical procedures with, or without the need for fillers.

The nose is the central most prominent part of the face and major contributor to facial balance. Nasalplasty is a technically and aesthetically demanding procedure as a small incremental change to your facial appearance can easily be noticed.

Surgery of the nose is one of the most common of all plastic surgery procedures. There are individuals who have been unhappy with the appearance of their nose their whole life, and there are those who have had their noses altered as a result of an injury. Rhinoplasty offers a solution to individuals who are looking for an improvement in the way they look and have realistic expectations for the procedure. There are many ways rhinoplasty can improve your nose’s appearance. The surgery can reduce or increase the overall size, change the shape, modify the shape of the tip or the bridge and narrow or widen the nostrils. In addition to enhancing your appearance, in some circumstances rhinoplasty can also help improve one’s breathing. The consultation is an important part of the process because it allows the doctor to examine the current state of your nose and determine what can be done to enhance its appearance as well as improve your breathing.

Eyelidplasty is done to remove excess skin and fat from the eyelids that make them look baggy, giving the patient a "tired" look. The operation may be combined with a canthoplasty, an eyelid tightening procedure, in cases where the eyelid is lax. Eyelidplasty is often performed in conjunction with a facelift and/or a brow lift to provide balance and harmony.

Chin Implants

Chin implants are made of silastic and come in a variety of sizes. They are placed either through an incision in the mouth or under the chin and act to increase the projection of a weak chin. The procedures are sometimes performed in combination with a rhinoplasty to enhance overall balance of the facial profile, or with a facelift to improve the contour of the jaw line or sagging chin.

In some cases, rather than using an implant, the front of the jaw bone can be cut and repositioned to improve the facial profile.

Otoplasty is a procedure designed to achieve more normal looking ears, to pin back protruding ears closer to the head or reduce the size of overly large ears. This procedure is mostly commonly performed on younger individuals to avoid being teased by others but many adults seek these improvements as well. The Otoplasty procedure varies with each patient and their degree of ear deformity.

The operation usually takes about two hours and is performed in an outpatient setting under general anesthesia, or local anesthesia with sedation. During the operation, an incision is made behind the ear to hide any obvious marks of surgical intervention. Multiple, strategically placed sutures are positioned to shape the cartilage into normal folds, which were missing pre-operatively. The cartilage is then reshaped using techniques of scoring, suturing, limited removal or combinations of the three.

The reshaped framework is then repositioned to achieve the best balance with the opposite side and to reduce excessive projection. The skin is redraped, any excess skin is removed and the incision behind the ear is closed.

To reduce the swelling or collection of fluid beneath the skin, you'll have a dressing put on your head to put gentle pressure on your ears and maintain their new position. After about two or three days, the dressing will be removed and you will be asked to wear an elastic band at night to protect your ears from accidentally being pulled while you sleep. Sutures will require removal in 7 to 10 days. You'll be able to wash your hair as soon as your head dressing is removed on the second or third day after surgery.

Most patients experience very little pain following their procedure. There will be slight discomfort or soreness— mostly when you bump or move your ears. This will be for about a week or two. You'll notice some swelling in the ears for approximately two to three weeks following your surgery. Most bruising will resolve in the first week or two. During this time, you can cover your ears with your hair.

Torn Ear Lobe Repair and Ear Lobe Reduction

Earlobes may have enlarged holes or be split as a result of heavy earrings or pulled earrings. Some patients may also desire reduction of overlying large earlobe. Treatment of these problems involves trimming of the edges of the enlarged holes, or split earlobes or removal of a wedge of excess skin, and repair with sutures. Re-piercing of the earlobe is done several weeks later.

BOTOX® Injections

There are individuals who would like to enhance their appearance, but do not want to undergo surgery and want as little downtime as possible. Many of them have turned to BOTOX® injections, a quick, virtually painless procedure. BOTOX® is a form of botulinum toxin that, when injected, temporarily weakens muscles that cause wrinkles. This treatment reduces or eliminates frown lines, forehead creases, crow’s feet near the eyes and thick bands in the neck. Dr. Valauri’s technique is to weaken the muscles to reduce the wrinkles, and yet allow them to function when you laugh, thus avoiding the unnatural, “frozen face” look. BOTOX® treatments result in a more youthful and refreshed appearance.

BOTOX® injections take minutes to perform, depending on the number of areas treated and the size of the area treated. With BOTOX® injections, there is little or no discomfort involved, and most patients are able to return to their normal activities immediately after receiving treatment. Generally, the effects of your BOTOX® injections may last up to 6 months and further treatments may be advised. BOTOX® is quite safe and has been used in high doses for decades for children with spastic muscle disorders, with very few known side effects. These include temporary bruising, and mild muscle aches. Because some wrinkles are not solely caused by the underlying muscles they may remain even after BOTOX® treatment. These wrinkles may be improved through the use of injectable fillers. For this reason a combination of BOTOX® and Fillers may be necessary to achieve an optimal improvement. Additionally, BOTOX® and Fillers may be used in patient’s after eyelid and facelift surgeries to enhance their results.

Injectable Fillers

Soft tissue augmentation with injectable fillers such as collagen, fat, Restylane™, Radiance™ and Sculptra™ injections can help restore youthfulness in one’s appearance without the incisions and the downtime of surgery. These injections result in an improvement of skin wrinkles and a more youthful appearance. This procedure is especially helpful in adding fullness to the lips and cheeks. When injected, the filler plumps up the creased and sunken areas of the face.

There is always a possibility of complications occurring with these procedures. With collagen, the risk involved is a small possibility of an allergic reaction. A skin test is performed to help reduce this risk. Collagen has been available for many years and thus, more is known about its potential risks and long-term outcomes. Collagen comes in different preparations for use for different areas of the face and can last up to four months.

The newer fillers on the market are reported to have been designed to be longer lasting than collagen, but there are reports of hypersensitivity reactions in some patients. Therefore these injectable fillers are very selectively prescribed.

Fat injections are another alternative and are not susceptible to allergic reaction since the fat is harvested from your own body. Filler fat injections can help reduce the appearance of laugh lines, crow’s feet, facial creases and can also be used to enhance the appearance of your lips.

By using mildly abrasive particles that gently scrub the top layers of the epidermis to remove the waxy keratin, microdermabrasion gives your skin a smooth and healthy appearance. This can be combined with AHA (glycolic acid) treatments to further enhance your results.

Laser Hair Removal

Permanent reduction of unwanted hair can be achieved by the use of special LASERS without the need for waxing or electrolysis. We are so confident in the safety and the effectiveness of our LASER technique that we offer a “patient satisfaction” program. Details of this program will be discussed during your consultation. In our experiences of over a decade we’ve found that a series of treatments is required for optimal results and that the best candidates are men and women with light skin and dark hair.
